2017 Conference Registration Form (Part 2)

Please fill out and submit Part 2 of the registration form to allow conference organizers to account for the venue, food, and accessibility needs. After submitting Part 2, your conference registration is complete. 

The BC Health Coalition has a limited amount of funds to help our members from around the province attend the provincial conference, but we also want as many people as possible to attend. 

Please let us know if you will require a travel subsidy to attend the two-day conference and an estimate of how much, for what purpose and whether you will need it ahead of time.

Travel subsidy can help cover transportation, accommodation, and food.

We are accepting payment on a sliding scale from $0 (no cost) to $75. No one will be turned away due to lack of funds. To pay online, visit http://bchealthcoalition.nationbuilder.com/bchc_conf_2017. To mail in payment, you can submit cash or make cheques out to "BC Health Coalition" and mail to: BC Health Coalition, 302-3102 Main Street, Vancouver, BC V5T 3G7. For those attending at no cost ($0), indicate so above and you do NOT have to submit payment online or mail.

Travel and accommodation subsidies are available for participants located outside of the Vancouver area
For estimating costs for mileage, the reimbursement rate is .54/kilometer driven. Please include return bus fares, or ferry costs for regular vehicle and passenger in estimation.
We can help cover $20/night for billeting with a friend or relative, if needed. Please try to find the most affordable accommodation possible - staff can help.
Please keep receipts of food/restaurant purchases while traveling.

Please note that while we enourage participation by individuals living outside the Vancouver area, we have a limited travel and accommodation budget and cannot guarantee that all applicants will receive subsidies. BCHC staff will contact you directly if you have requested travel and accommodation assistance.
